We are proud to offer you the
most current treatment options and materials available to give you a smile
that is attractive, comfortable, and long-lasting. To protect your
investment, proper home care and regular visits are a natural follow-up to
in-office care. We are pleased to offer you a warranty on your
dental treatment. To maintain this warranty, please be sure to read
the warranty conditions.
Composite (tooth-colored) Fillings
When a composite restoration is done, we will replace or repair it in
the event of failure up to 3 years at no cost to you. If the tooth
breaks and more extensive treatment is required than another composite,
the original fee will be applied to the future treatment.
Crowns, Bridges, Onlays, Inlays, and
Porcelain Veneers
We will warranty these most comprehensive procedures up to 3 years. If
necessary, we will replace or repair them at no charge if they break.
This does not include accidents that could also break normal, healthy
teeth. This warranty is null and void if the whole tooth breaks off.
Root Canals
Root canal therapy is about 96 percent successful; however, root canal
therapy does occasionally fail. If this should occur within 3 years,
we will re-treat the root canal, or apply the original fee paid as a
credit toward a replacement tooth or prosthesis.
Dentures and Partial Dentures
We will warranty dentures and partials for a period up to 3 years, and the
use of our denture cleaner is required. If a tooth on a denture or
partial chips or breaks during the first 3 years, we will repair it at no
charge. This does not cover accidents such as dropping your denture.
Note: Relines may be recommended every year, in order to preserve your
denture and oral health. Relines are not covered under this
warranty. Summary
We are confident of the durability of the materials and procedures
prescribed for you. We warranty all comprehensive treatment for 3
years, depending on the treatment performed, from the date of placement.
We will repair or replace these, at no charge to you, for the full 3 years due to breakage due to normal use. If more extensive treatment
is required, the original fee will be applied to the future treatment.
Our assurance to you does not include accidents or trauma which could
cause damage to the teeth or dental prosthesis. Failure to have your
regular re-care visits, at the prescribed intervals with our office, voids
all warranties. Help us to help maintain your teeth for a lifetime.
